A team at Bethesda created the vocabulary, grammar and writing system of Dovahzul. We know how to translate these because of in game sources such as the shouts section or books and external sources like the prima games guide. 

Occasionally however you will also see words made by fans. These are non-canon (aka made up) and aren't commonly used anymore.
